Family First Health is a National Committee for Quality Assurance (NCQA) recognized Patient Centered Medical Home (PCMH). Through this model, we provide team-based care with a focus on wellness and prevention. The medical team including your provider, nurse, medical assistant and medical receptionist pre-plan for your visit and review what is needed to make sure your medical visit includes all aspects of care for good health outcomes. Your medical team will engage other care team members as needed, depending on your specific needs. This may be a referral to specialty providers, lab orders, additional screenings or tests, or in-house referrals for integrated behavioral health and substance use services.

Family First Health is proud to offer board certified primary care physicians (MDs and DOs), Physician Assistants (PAs), and nurse practitioners (NPs) at our health center medical locations. Please ask us about our additional specialty providers in the areas of pediatrics, women’s health, psychiatry, and HIV care. Whether your health care concerns are chronic – such as managing diabetes or hypertension; acute – such as managing a temporary infection; or preventive – such as discussing your exercise and nutrition goals, our team uses the most current evidence-based guidelines, which are programmed into our electronic medical record, Athena.
Athena can help your care team coordinate your health care based on the most up-to-date guidelines for providing quality of care, as defined by the U.S. Preventive Services Task Force, HEDIS, and HRSA’s Universal Data System (UDS) Clinical Quality Measures based on Healthy People targets.
Athena also enables you to self-manage your health care through its patient portal. Within the portal, you can:

How do I make an appointment?
If this is your first appointment, please call the center of your choice and talk with one of our team members. We will provide information on which of our providers are available to see new patients. If you are already an established patient, you can call the office to schedule your next appointment or use our patient portal. Both new and established patients can now also self-schedule using our online registration tool. Simply follow this link or click the “Schedule Appointment” button at the top of any page on our website.
What should I know/bring for my first appointment?
- Photo ID/driver’s license
- Insurance card
- A list of any questions you may have for your provider and care team
- Name/address of recent health care provider (So we can send a signed release for medical records for your new provider to review with you)

Will all of my questions and health care needs be taken care of at my first medical visit?
We will do our best to take care of your questions and health care needs at your first visit. Please be prepared to tell your new provider what are the one or two most important concerns. If additional time is needed , you will be scheduled for a follow-up visit as soon as possible.
What if I have a question or need care after-hours?
If you are experiencing an emergency after-hours, please call 911 or go to your nearest emergency department. If you need assistance with non-emergency clinical concerns after-hours, please call your practice location. You will be connected to our nurse call service or the provider on-call.
How will I know if you accept my insurance?
When you call to schedule your first appointment, we will ask for your insurance information and should be able to confirm right away. If not, we have several options available, including our sliding fee discount. You may also check our full list of accepted insurances.
If I don’t have insurance, can I be seen?
Of course! We will see anyone, regardless of ability to pay. There are several options available including our sliding fee discount. If you are interested in looking for insurance through the Health Insurance Marketplace, we will schedule time for you to meet with our insurance navigator who can walk you through that process.
