FFH is a recognized Patient-Centered Medical Home and provides team-based care with a focus on wellness and prevention. Our providers meet patients’ needs through ongoing, consistent preventative care and deliver the full range of services for patients.

NFP partners first-time mothers with a registered nurse from early pregnancy through the child’s second birthday. Regular home visits provide expectant mothers with a relationship they can count on, allowing them to develop confidence and skills for parenting.
Good oral health is essential to overall health and a lifetime of healthy smiles. Family First Health provides comprehensive dental services for the whole family and supports patients in not only achieving good dental health, but maintaining it.
An initiative of Family First Health, our School Mobile Dentist Program is a mobile pediatric oral healthcare system designed to allow our dental team to provide services in schools and childcare centers, including local Head Start and Pre-K Counts programs.
Caring Together provides coordinated, high-quality care for people who are HIV-positive so they can easily access the services that they need. Caring Together also provides free HIV testing for individuals age 13 and older at the George Street Center, Monday-Friday from 8:30am-3:30pm.
Connections For a Healthy Pregnancy partners expectant mothers with a Community Health Worker through pregnancy. Regular meetings help expectant mothers navigate the health care system, identify and connect with resources for a healthy pregnancy, and adopt healthy behaviors.
